I was creating a campign at work today and while searching for quick ways of adding phrase and exact match keywords to your broad keyword list, I couldn’t find anything on it. Since I was using excel, I found a way of making this task much, much easier than adding in the [ ]’s and ” “’s manually!
The first step is to create your keyword list in excel as shown below:

To the right of the main keyword column, you can see that I have the phrase match copied in. I achieve this easily by using this formula:
=”["&A149&"]” (where A149 equals the cell of the broad term you want to use)
This tells excel to copy into the cell the broad match term and add a “[" and "]” to the end of the word. Copy this down the list to replicate.
Next we need the exact match variation. Unless I’ve missed something, Excel doesn’t allow the same method to use quotation marks within quotation marks… It messes up.
So… Copy the phrase match list (the [ ] ones) and copy them into Notepad. Now press CTRL+A (select all) and then CTRL+H (Find and Replace). Insert ‘[‘ into the find box, and ” into the replace with box:

Click ‘Replace All’ and then repeat using the other ‘[‘ to change the other side. This whole process takes under a minute and can save you a lot of time generating phrase and exact match variations of your broad match PPC keyword lists.

That’s a great method. I personally use PPC Keyword Toolz. I can create huge campaigns with hundreds of adgroups very quickly.
I agree with Angela, i have been using PPC key word toolz since its release, it blows everything else out of the water. I also has Dynamic Keyword Insertion
